Basic Infant Information Form

This form will help us get to know your child's feeding schedule at the beginning of their time with us. Naturally, their schedule will change and you're welcome to keep the teacher informed of any updates on the daily sheets provided in the room.

Child Medical Statement

Each child must have a medical statement on file on their first day of enrollment. The medical statement must be updated every 13 months.

Child Medical/Physical Care Plan

For children with specific medical conditions.

Infant Sleep Waiver

Swaddling bags and devices designed to maintain sleep positions are permitted but must be prescribed in writing by a physician and be available on file for review.

Photo Release Form

From time to time teachers take pictures of your children during various activities. We would love to be able to show them off on our website. In order to do this we need your permission. Please fill out the Photo Release Form and turn it into the office.

Request to Administer Medication

To be used for medications or diaper cream. Please fill out completely.

Student Withdrawal Form

Our withdrawal policy states:

  • Full-day students: Parents planning on withdrawing their children from the full-day program should do so by submitting a withdrawal form prior to the 15th of the month. There will be no penalty, although the full month’s tuition will still be due.
  • Half-day students: Parents planning on withdrawing their children from the half-day preschool program should do so by submitting a withdrawal form prior to the 20th of the month. The child’s last day will be the 15th of the following month. There will be no refund for missed attendance during this time.
  • If you withdraw your child and re-enroll them in the same academic year, you will need to again pay the $65 re-enrollment fee.

    Vacation Credit Form

    Grace ECC allows a one-week vacation credit for our year-round, full-day families. You must be enrolled full time for 12 consecutive months before you are able to request this credit. Vacation credits for subsequent years will be applied no sooner than 12 months after the previous request. You will receive one credit per 12-month period for the program option you are enrolled in. This credit is not available for our 9-month or half-day families.
